An example Princeton Analytical Labs Chain of Custody Form is included for review. The form is used to accompany samples to the laboratory to outline the testing requested. The form is self-explanatory but we suggest contacting the laboratory with any specific questions. Once the sample and chain of custody request form are received at the laboratory, the Princeton Analytical Labs staff will review for completeness.
